The challenge facing travel agents today

Your margins are shrinking. Your clients need more. Online booking platforms have compressed travel agent commissions for years — meanwhile, your clients are asking about services you don't currently offer.

Flight and hotel commissions squeezed by OTAs — your margins per booking get thinner every year

Clients asking about medical treatment abroad — and you're sending them to someone else (or nowhere)

High-value clients travelling for healthcare without you — revenue you're leaving on the table

Keep your travel commission. Add treatment commission on top.

You book the flights, hotels, and transfers — and keep that commission as you normally would. M1H pays you an additional 5–15% of the patient's total treatment cost. Two revenue streams from the same client.

Example: Cardiac surgery patient travelling to Turkey

Your Travel Commission

$150–$300

Flights, hotel, transfers (your normal earnings)

M1H Treatment Commission

$1,200–$2,500

5–15% of treatment cost (paid by My 1Health monthly)

Total Per Client

$1,350–$2,800

5–10x what you'd earn from travel alone

Illustrative example based on typical flight/hotel commissions and blended 8–10% M1H treatment commission. Actual rates confirmed during onboarding.

What the stacked commissions look like across procedures

Procedure (Destination) Your Travel Comm. M1H Treatment Comm. Total Per Client
Cardiac surgery (Turkey) $150–$300 $1,200–$2,500 $1,350–$2,800
Knee replacement (India) $100–$200 $500–$900 $600–$1,100
Oncology treatment (Thailand) $150–$300 $1,500–$4,000 $1,650–$4,300
Fertility / IVF (UAE) $100–$250 $400–$1,200 $500–$1,450
Dental implants (Mexico) $50–$100 $200–$500 $250–$600
Cosmetic surgery (Turkey) $100–$200 $300–$800 $400–$1,000

Illustrative ranges based on typical travel commissions and blended 8–10% M1H treatment commission midpoint. Actual rates confirmed during onboarding with your RPM.

You handle the travel. We handle the healthcare.

Clear division of responsibilities. You stay in your zone of expertise. We handle the medical side.

What You Do
  • Identify clients who need or want healthcare abroad
  • Introduce the M1H hospital network and treatment options
  • Book flights, hotels, and ground transfers (as you normally would)
  • Submit the patient inquiry through the RPS platform
  • Stay the client's main point of contact throughout the journey
What M1H Handles
  • Hospital matching, treatment plans, and cost estimates
  • Medical record transfers and appointment scheduling
  • Hospital coordination and clinical case management
  • Commission calculation, reconciliation, and monthly payment
  • Case escalation support if clinical complications arise

Your dedicated RPM guides you through your first cases step by step. Many agents start with wellness retreats, dental, or cosmetic referrals — familiar travel services with healthcare commissions attached — before expanding into complex medical procedures. You're never expected to make clinical decisions.

Here's what this looks like in practice:

UK agent

A corporate client mentions their finance director needs a knee replacement but faces a 14-month NHS wait. You introduce the M1H option — JCI-accredited hospitals in Turkey and India with 2-week wait times. You book the flights and hotel as normal. M1H handles the hospital match, treatment coordination, and medical records. The client gets treated. You earn $600–$1,100 instead of the $100–$200 you'd have earned on a standard business trip.

Australian agent

A couple planning a Bali honeymoon asks about combining their trip with a smile makeover. You suggest an M1H-partnered dental clinic offering porcelain veneers and whitening at a fraction of Australian prices. You arrange flights and the resort as usual. M1H handles the clinic match, appointment scheduling, and treatment coordination. Both procedures are completed before the holiday begins. You earn $500–$800 on top of your standard travel commission.

Nigerian agent

A client mentions they've been trying to conceive and asks about IVF options overseas. You introduce a JCI-accredited fertility clinic in Turkey offering full IVF cycles at a fraction of local private costs. You arrange flights and hotel accommodation as normal. M1H handles the clinic match, cycle scheduling, and all medical coordination. The client completes treatment and returns home. You earn $800–$1,400 on what began as a routine travel inquiry.

Frequently Asked Questions

Have a question? Our team is available to help.
Do I need healthcare experience to offer this?

No. You handle the travel side — flights, hotels, transfers — as you normally would. M1H handles the medical side — hospital matching, treatment coordination, clinical management. Your RPM guides you through your first cases. You never need to make clinical decisions.

How does the commission stacking work?

You keep 100% of your normal travel commissions (flights, hotels, transfers) — these are booked through your existing channels as always. On top of that, M1H pays you 5–15% of the patient's total treatment cost. The two commissions are completely separate. You earn both on the same client.

Do I need the CHWTP certification?

No — it's optional. Many travel agents join as add-on service partners with a quick onboarding process and no certification requirement. The CHWTP certification ($199) is available if you want deeper training and a professional credential — it's especially valuable if you want to build healthcare travel into a core service offering rather than an occasional add-on.

What types of clients are good candidates?

Any client who might benefit from medical treatment abroad: people facing long wait times for elective procedures, people who can't afford treatment in their home country, people seeking specialist care not available locally, people interested in wellness retreats or cosmetic procedures, or corporate clients looking for executive health screening packages. Many agents start with wellness retreats and cosmetic referrals — familiar travel services with healthcare commissions attached — before expanding into medical procedures. Your RPM will help you identify healthcare opportunities in your existing client base.

Does this change my existing travel business?

Not at all. Your existing travel business continues exactly as it is. Healthcare is an additional service — you don't rebrand, restructure, or redirect your existing clients. Some agents offer it to every client. Others offer it only when a client specifically asks about medical travel. The level of integration is entirely up to you. If you work under a host agency, M1H treatment commissions are paid directly to you — they don't flow through your host agency unless you arrange it otherwise.

How and where do I get paid?

Treatment commissions are paid by M1H to 160+ countries in USD or your choice of 140+ local currencies. You also get a built-in USD wallet. Travel commissions come through your normal booking channels — those are completely separate and unaffected. M1H treatment commissions are reconciled monthly with full transparency through the RPS platform.

What if something goes wrong during a patient's treatment?

All hospitals in the M1H network carry professional indemnity insurance and meet rigorous international accreditation standards (JCI, NABH, TEMOS). If a clinical complication arises, M1H provides case escalation support — working directly with the hospital. You handle the travel rebooking if needed. You're never expected to manage clinical issues — that's M1H's responsibility.

I run a host agency / travel network. Can my agents join?

Yes — we work with individual agents and agency networks. If you manage a team of agents or a host agency network, talk to our team about a group partnership structure. We can onboard multiple agents under your agency with centralised reporting and commission tracking.

Your clients are already travelling for healthcare. Are you earning from it?

Keep your travel commissions. Add 5–15% treatment commissions on top. 350+ hospitals. One platform. A dedicated partner manager.
